The Nurse Practitioner is an expert in inpatient diabetes care and contributes to excellence in patient care and teaching Responsible for: Nursing and Provider education related to diabetes care and insulin safety; delivery of evidence-based inpatient diabetes care; outcomes measurement (tracking, analysis and reporting of clinical, and satisfaction outcomes); and participation in Joint Commission Certification in Advanced Diabetes Care. As a member of the inpatient glycemic management team, provides direct patient care and works collaboratively with the medical team to establish priorities and to assess solutions Job Responsibilities: Collaborates with all professional staff in planning, developing and implementing in-service diabetes training sessions for staff to ensure clinical and technical competency Collaborates and works closely with Physicians, Nursing, Pharmacy, and other departments to assess current processes, quality of care and barriers to necessary changes in clinical practice related to inpatient diabetes care Engages in care management by coordinating with Case Managers, Social Workers and Providers regarding discharge plans Provides regular or episodic care and education directed to patients with diabetes in which there are recurrent problems, and then collaborates with other members of the health care team to develop and implement pathways and quality improvement plans Champions standards of care relating to Joint Commission Advanced Diabetes Certification Collects and interprets glucometrics that direct changes in care processes. Participates in clinical care through order entry, when appropriate, and oversight of glycemic management Performs other duties as assigned Minimum Qualifications: Required Licenses: Preferred Qualifications: Qualified candidates must be able to effectively communicate with all levels of the organization.
